What to Bring
What to Bring
What to expect when you come in
Your initial appointment will last an hour and a half – far longer than visits to other doctors’ offices. You will first meet with one of our staff members for an orientation to the office and services. She will answer questions regarding clinic policies and operations, and will assist you with completing your forms. You then will meet with Dr. Varela for your initial evaluation and physical examination. He will review your neurological condition, starting from its earliest onset. He will also review your previous treatment, testing, and your medical history, as well as any additional concerns you may have. The goal is to make sure that we have a thorough understanding of your unique pattern of disease. From this foundation of understanding, we then develop together a plan of care that will include any necessary diagnostic testing, as well as treatment recommendations specific for you. At the end of your visit, our staff will help you schedule any tests and referrals, and arrange for your follow-up visit to the office.
What to bring
Bring a list. Please write down any questions or concerns that you may have, and bring this list with you to the appointment. That way, we can be sure to cover what is most important to you while you are here. You may also consider bringing someone with you – a spouse, significant other, confidant, or friend. While not a requirement, many clients find it helpful to have someone along to help you remember what you wanted to say, and can confirm for you later what was said at the appointment.
Medical Records
• Please bring any available medical records that relate to your disease, and that you feel would be helpful. Examples: Copies of test results: such as Labs/blood work, MRI, CT scan, Sleep Studies, EEGs, EMG, Echocardiograms, other imaging studies.
• Copies of pertinent procedure/surgery notes: such as injections, brain or cervical spine surgeries.
Current Medications
It is very important that we receive a current and accurate list of all your medications (including dose, frequency, prescribing doctor). Please also include separately, a list of all over-the-counter medications and supplements. Many clients find it helpful to bring the bottles.
Insurance Card
Visits to Novel Clinical & Neurology may be covered by your insurance plan under the Out-of-Network benefits provision.
