Dr. Heber Varela has 20 years of experience in general adult neurology with special interest in electromyography (EMG) and neuromuscular disorders.

Dr. Varela is a former Assistant Professor of Neurology and Associate Program Director at the University of South Florida (USF). He has served as Principal Investigator in multiple NIH and industry-sponsored clinical trials for Alzheimer’s, ALS, Parkinson’s, and Multiple Sclerosis.

Any previous neurological evaluations, lab results, or imaging studies (MRI, CT scans).

A complete list of all current medications, supplements, and dosages.

Family History

Information about neurological conditions in your family, if known.

Symptom Notes

Write down your symptoms, when they started, and anything that makes them better or worse.
